I have a theory that I would like to share. My theory is that Matteo is actually adopted. I have a couple different scenes to prove my point. The first scene that I want to talk about is the scene where Matteo talks to the girl with the camera. During that scene, Matteo points out key visual things and makes conclusions about their life stories. For example, He sees an old woman wearing two wedding rings and deduces that she is a widow and is lonely. While this scene might be used to give insight into Matteo's character I believe that it has another purpose. The other purpose being that the director wants to encourage the viewer to use the same visual intuition that Matteo does during this scene. He even says to the woman that she has to "look at the soul".
Now that the viewer is encouraged to make conclusions about the character's life stories through visual observation, there are some immediate differences between Matteo and his family. He has different eye color, hair color, and overall general appearance. He is also a lot more quick to anger especially compared to his brother Nicola. Now this not might necessarily mean that he is adopted, it could just be the actor chosen for the movie. However, this point is quickly disproven by the fact that Matteo's appearance is mentioned several times during the movie and people often point out how different he looks from Nicola.
In another scene where Matteo's mother is talking to Nicola's wife Giulia about motherhood, she mentions how she feels like she failed Matteo and how she wishes she treated him differently than she did. Now this also doesn't really prove that Matteo is adopted at all. However, I think that since the director wants us to use subtle details to try and derive meaning from them that this can just be used as another point of evidence.
Overall, these things all together form my case for why I think that Matteo is adopted. To summarize, the director uses the scene where Matteo talks to the woman taking pictures to encourage the viewer to hone in on small details and make deductions about that character's life. We can apply this principle to the fact that Matteo is different both in appearance and personality to the rest of his family. We can also apply this principle to the scene where Matteo's mother reveals that she feels like she failed him and wishes that she treated him differently. In conclusion, I believe that these details are included in the movie because the director wants the viewers to conclude that Matteo is adopted.
